Risk Management seeks information from
aviators
Risk Management currently is reviewing its non-owned aviation
liability insurance. Departments with employees who use their own
aircraft or lease aircraft to conduct university business are
requested to send the following information by May 1 to Risk
Management at 651 Serra St., Room 250, mail code 6207; fax number
723-9456:
- Plane identification by year, make and model; hours flown in
the past 12 months; and pilot certification type (an online form is
available at
www.stanford.edu/dept/Risk-Management/docs/forms/planeuse.fft).
- A signed release from liability (online at
www.stanford.edu/dept/Risk-Management/docs/forms/pilotwaiver.shtml).
- A certificate from the insurance carrier with evidence of
aircraft liability coverage with minimum liability limits of $5
million, with no sublimit for bodily injury liability and no
sublimit for passenger liability. The certificate must name
Stanford as an additional insured and include a waiver of
subrogation in favor of the university.
